Symptoms Questionnaire

The questionnaire below is one of the most widely used methods to assess menopausal symptoms.

It is called the Greene Climacteric Scale and covers most of the common symptoms of perimenopause and menopause. This is a verified measurement tool used globally in both clinical practice and research studies.

However, it cannot be used on its own to diagnose menopause, and it does not cover every possible symptom—including some vaginal and urinary changes.

The scale can form a useful part of an overall assessment and can also be used to track how your symptoms change over time. This form is useful to take to your GP to start a discussion about your menopause symptoms.
